    What this test measures

    The PHQ-9 screens for depression symptoms such as low mood, loss of interest, sleep or appetite change, fatigue, and difficulty concentrating. It is public-domain, validated in many countries including India, and takes a few minutes.

    How often nine core depression symptoms have bothered you over the past two weeks. Question 9 asks about thoughts of self-harm. Any yes on that item deserves prompt support, regardless of the total score.

    How scoring works

    Each item is scored 0–3. Totals range from 0 to 27. A score of 10 or higher often indicates clinically significant symptoms that are worth discussing with a professional. This page is a screen, not a diagnosis.

    0–4 · Minimal

    Few symptoms in the past two weeks.

    5–9 · Mild

    Some symptoms. Self-care and a check-in with someone you trust can help.

    10–14 · Moderate

    Symptoms are likely affecting daily life. Speaking with a therapist is recommended.

    15–19 · Moderately severe

    Professional support is strongly recommended.

    20–27 · Severe
